A North Staffordshire haulage company has been hit with a fine and costs totalling pounds40,000 for health and safety violations following the death of one of its drivers.

Martyn Simm, pictured, was closing up at Berser International Cargo Services after finishing his deliveries when one of the metal gates fell on top of him, Stafford Crown Court heard yesterday.
